Multivitamins: Who Needs One and How to Choose

A daily multivitamin is the most common supplement in American homes. Here is what it can and cannot do, who is most likely to benefit, and how to choose one.

What is a multivitamin?

Vitamins and minerals are essential substances the body needs to function normally. A multivitamin/mineral supplement combines many of them in one tablet, sometimes with added fruit and vegetable blends, enzymes or trace minerals.

What the research says

According to the National Center for Complementary and Integrative Health (NIH), most studies have shown little or no effect of multivitamins on the risk of health problems, and a multivitamin cannot take the place of eating a variety of foods. The same source notes that a basic daily multivitamin is unlikely to pose a health risk for healthy people. Think of it as a way to fill gaps, not as a shortcut.

Who may benefit?

  • People on low-calorie diets or who avoid certain foods.
  • People whose healthcare provider has recommended a supplement for a medical reason.
  • Anyone whose everyday diet is short on fruit, vegetables or variety.

How to read the label

  • % Daily Value. Around 100% for most nutrients is plenty. Very high percentages are not better.
  • Serving size. Check whether the amounts are for one tablet or several.
  • Diet and allergens. Look for vegan, gluten-free or non-GMO statements if they matter to you.
  • What you already take. Add up nutrients across all your supplements and fortified foods.

Safety points to know

  • Pregnancy. Too much preformed vitamin A during pregnancy has been shown to increase the risk of birth defects. Use a prenatal formula recommended by your provider.
  • Blood thinners. If you take warfarin, talk to your healthcare provider before taking any supplement that contains vitamin K.
  • Smokers. Smokers, and possibly former smokers, should avoid multivitamins with large amounts of vitamin A or beta-carotene.
  • Children. Iron supplements are a leading cause of poisoning in young children. Keep all supplements out of reach.
